About the role
n
Working within the Care team, you'll collaborate directly with participants, families and stakeholders to deliver high-quality assessments, personalised care planning and comprehensive case management, which promotes participants goals and wellness outcomes.
n
Key responsibilities:
n
Conduct HCP assessments and reviews in participants' homes
n
Convert client needs into funding opportunities to maximise package utilisation
n
Build strong relationships with participants, support networks and internal teams
n
Deliver high-quality, person-centred care
n
Provide comprehensive case management and advocacy
n
Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports
n
Manage incidents,



feedback and complaints in line with ACQS requirements
n
Record and maintain high quality, comprehensive documentation and case notes
n
Travel is required with km reimbursement provided
n
About you
n
Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability), with ideally 3+ years industry experience
n
Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions
n
A natural influencer who engages stakeholders and drives alignment and action
n
Highly organised, agile and adept at managing competing priorities
n
Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways
n
Committed to continuous improvement and service excellence
n
Balances care outcomes with financial sustainability
