Job Description
Beyond Choice is looking for a Support Coordinator to join our team in Queensland. This is a contractual position, offering flexible work arrangements to suit your lifestyle. You will develop and implement comprehensive support plans to meet the individual needs of NDIS participants, coordinate access to a range of support services, and advocate on behalf of clients to ensure they receive the appropriate level of funding and support.

Key responsibilities

Develop and implement comprehensive support plans to meet the individual needs of NDIS participants
n
Coordinate access to a range of support services, including allied health, accommodation, and community engagement
n
Advocate on behalf of clients to ensure they receive the appropriate level of funding and support
n
Regularly monitor and review client progress, making adjustments to support plans as needed
n
Maintain detailed records and provide regular reports to clients, their families, and the NDIS
n
Collaborate with a multidisciplinary team of health and community professionals
n
Contribute to the continuous improvement of NDIS support services
n
About you
n
Minimum 2 years' experience in NDIS support coordination or a related field
n
Preferred to have established caseload.
n
Thorough understanding of the NDIS framework and eligibility criteria
n




Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of clients
n
Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients
n
Demonstrated ability to work independently and as part of a team
n
Relevant tertiary qualification in disability services, social work, or a related field
n
Current driver's licence and access to a reliable vehicle
n
