About Us
Nepean Blue Mountains Local Health District (NBMLHD) is an exceptional environment to advance your career while enhancing your skills and expertise.

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers.
Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care.
Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
Join our dedicated Community Health team based at Lithgow Community Health Centre for this full time opportunity.
As a Registered Nurse, you will provide high-quality, person-centred nursing care to individuals, families and carers across a variety of community settings, including clients' homes and community clinics.
Working within a multidisciplinary team, you will deliver a broad range of services including wound management, palliative care,



chronic disease management and continence care.
The successful incumbent will be responsible for assessing, planning, implementing and evaluating care using evidence-based practice and a primary health care approach, while partnering closely with clients, carers, GPs and other healthcare providers.
This role offers the opportunity to make a meaningful difference in the community by supporting clients to maintain their health, independence and wellbeing.
You will also provide supervision and support to Enrolled Nurses, ensuring safe and effective delegation of clinical care.
This position is based on a 7-day rotating roster and uniform is required.

What you will bring to the role
Current registration as a Registered Nurse Division 1 with AHPRA with demonstrated experience and clinical skill in general/community nursing as a Registered Nurse including completion of a new graduate Registered Nurse program or equivalent experience working as a Registered Nurse.
Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services and an ability to liaise with other health professionals to facilitate case management and care planning.
Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
Demonstrated effective communication skills (written and verbal) and interpersonal skills.
Demonstrated commitment to professional development, evidence-based practice and an awareness of changes within the health care setting.
Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
Ability to work a flexible roster, across all days of the week according to the team requirements.
Current valid NSW State driver's licence (including P1 or above) and willingness to use for work purposes.
This may include driving long distances across the LHD.
