The Opportunity:

Melbar is looking for an energetic and driven full‐time Care Partner to join our supportive and dedicated team.
n
Our Care Partners play a vital role in assessing, coordinating and managing care services for our Support at Home clients. As part of a vibrant and dynamic team, the Care Partner will ensure clients receive a holistic and person‐centred approach to care. With a focus on each client's individual needs, they will provide comprehensive, safe and effective support to help achieve clients' health and wellbeing outcomes.

Experience and Skills:
n
n




Relevant qualifications in Care with Community experience e.g. Certificate IV in Aged Care and/or Home and Community Services, Care management, Social Worker or Nursing
n
Demonstrated experience in a community/ care partner / nursing role;
n
Coordinating assessments & care planning;
n
Strong problem‐solving and leadership skills;
n
Experience in care management and the Aged Care Quality Standards
n
High level of organisational skills and details orientated;
n
Excellent verbal and written communication skills;
n
Bilingual skills are essential with Mandarin as a compulsory language;
n
Valid Australian driver's license with access to own vehicle;
n
Competent computer skills in Microsoft Office software and ability to learn business systems quickly;
n
Demonstrate familiarity with Support At Home program, having previous experience in a home care environment will be a bonus.
