Job Description
Wellways Australia Limited is seeking a Health Linker in the Murrumbidgee region to help people access health, community and social supports and to improve wellbeing and recovery goals.

You will collaborate with GPs and other health professionals to build coordinated pathways, support clients, and promote person-centred, recovery-oriented practice across multidisciplinary teams; a tertiary qualification in Social Work or related field is required and a willingness to travel will be needed.
