Job Description
NSW Health, Concord Repatriation General Hospital, is seeking a Clinical Aged Care Assessor (Social
Worker/Physiotherapist/Occupational
Therapist Level 1/2 or Registered Nurse) for a temporary full-time role. The position involves triaging referrals, conducting comprehensive assessments and coordinating care for frail older people, under the Aged Care Act ****.

The ideal candidate will have robust communication skills, ability to screen referrals, and hold a valid NSW driver's licence.
