Intake & Service Delivery Lead - Support at Home

As the Support at Home Lead – Driving Intake & Service Delivery, you will work closely with the existing Support at Home Team to ensure clients experience a smooth transition from initial referral through to commencement and ongoing delivery of services. You will provide hands‐on operational leadership, helping ensure services are coordinated effectively, client needs are understood and communicated, operational issues are addressed promptly and budgets and financial targets are supported and maintained.

Key responsibilities

Coordinating referrals, enquiries and client intake processes
n
Supporting client onboarding and commencement of Support at Home services
n
Ensuring client information, service requirements and documentation are accurate and complete
n
Drafting agreements and tracking budgets and plan spendings
n
Supporting effective service scheduling and workforce coordination
n
Monitoring service delivery and responding to changes in client needs
n
Identifying and resolving service delivery issues before they impact clients
n
Maintaining accurate client and service records
n




Supporting compliance with aged care requirements, organisational policies and service standards
n
Monitor service performance, capacity and operational priorities
n

About you
n
n
Experience within aged care or home care industry desirable
n
Experience in client intake, service coordination, care coordination or operational service delivery
n
Strong organisational and prioritisation skills
n
The ability to manage competing client and operational requirements
n
Strong communication and relationship-building skills
n
Sound judgement and the ability to resolve day-to-day service delivery issues
n
Strong computer literacy and accurate record-keeping skills
n
An understanding of quality, compliance and client-centred service delivery
n
The ability to work collaboratively while also taking ownership of your responsibilities
n

Salary depending on experience
