JOB DESCRIPTION

You'll support adults with chronic and complex health needs to build independence and improve their quality of life through practical, person-centred care.
n
You'll partner with clients, carers and other health professionals to deliver evidence-based services in home and community settings. This includes:
n
n
Completing comprehensive assessments, care planning and home visits
n
Recommending assistive technology, equipment and home modifications
n
Supporting clients to navigate aged care and community funding pathways
n
Delivering education, coaching and self-management strategies
n
Collaborating with a multidisciplinary allied health team
n
Participating in supervision, case reviews and continuous improvement initiatives
n
Maintaining high-quality documentation and contribute to service outcomes
n

Current AHPRA registration

Experience or knowledge in functional assessments, home modifications and assistive technology
