The Role: As our Community Worker, you will deliver community-based support services and implement programs aimed at improving the social, emotional, and practical wellbeing of individuals and families. Responsibilities:

Assess clients' needs and identify appropriate support services in line with individual circumstances
n
Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders
n
Provide information, guidance, and referrals to community services, including health, housing, and social support services
n
Support clients in accessing community resources, programs,



and government services
n
Liaise with healthcare providers, social workers, and other service agencies to coordinate care and support outcomes
n
n
Skills & Attributes:

Relevant qualification
n
Minimum 1 year of relevant experience
n
Solid interpersonal and communication skills
n
Ability to work independently and as part of a team
n
Demonstrated empathy, patience, and commitment to client wellbeing
n
Knowledge of NDIS framework and person-centred care principles (preferred)
n
n
