and identify bottlenecks or resourcing issuesMaintain accurate records and data across systems in line with policy and guidelinesCollaborate with internal stakeholders to support timely, high-quality service deliveryUse dashboards and reporting tools to optimise workflow and performanceContribute to continuous improvement of triage and allocation processesAs required, conduct assessments or support plan reviews via phone, telehealth, or in personThis is a Permanent Full-Time position, working Monday to Friday.You'll thrive in this role if youBring warmth, professionalism, and clinical insight to every interactionHold a tertiary qualification in a health-related discipline (e.g. nursing, OT, physio, social work) and current AHPRA registration or eligible to be part of a relevant professional association.Have a strong understanding of the Aged Care Act and aged care pathwaysCommunicate clearly and kindly, especially when navigating complex or urgent situationsAre confident using technology, including the My Aged Care portalWork well independently but thrive in a collaborative teamAre flexible, responsive, and ready to adapt to changing needsHold a current First Aid & CPR certificate, driver's licence, and are willing to use your own vehicle for workHave experience working across cultures and with vulnerable communitiesBelonging Starts HereWe believe real care starts with real belonging.
