We want our clients and families feeling they are seen, heard and known.This is a unique opportunity for an Occupational Therapist looking for a varied and autonomous role in Community Aged Care on a Full time or Part-time permanent basis.Your role will include visiting clients in their homes, providingcomprehensive Occupational Therapy (OT) assessments and interventions that promote health, wellnessand independence whilst supporting them to continue living safely in the community.Day to day this role looks like:Conduct comprehensive OT assessments for clients, including those with multiple and complex needs.Provide specific OT services and programs, including home assessments and advice on activities of daily living with recommendations involving home modifications, equipment prescription,



care planning and educationFacilitate and assist in development of a range of community based Health and Wellness programs and groupsInvolve clients and carers in developing care plans that are person centred and reablement focused.Provide support, education and direction to community workers and staff to assist them to best support our clientsSubmit individual applications for home modifications and equipment in accordance with program guidelinesWork collaboratively withthe wider allied health team,including nurses, physiotherapists, fitness instructors and allied health assistantsFace to face clinical work and minimum travel time between clients with time allocated for administration and reportingOngoing liaison with external agencies and suppliers to arrange services and products required by the clients in accordance with policies and proceduresTo be successful in this role you'll need:Tertiary qualification in Occupational Therapy with post graduate experienceCurrent AHPRA registrationCommitment and enthusiastic approach towards working with the clients, families, carers and advocates.Experience in equipment prescription, pressure care, home modifications and falls prevention for older adultsIndustry knowledge of local and international wellness and reablement models of careExperience working in a community setting and demonstrated knowledge of the aged and disability service system in the community health sector - desirable but not essentialCurrent Drivers Licence and registered and insured motor vehicleWillingness to apply for a Police CheckWhat we can offer you:Paid travel time & KM's reimbursed ($1.11 per km)Not-for-profit tax benefits so you pay less taxAccess to professional development and clinical support (up to $2,000 CPD allowance per year)Fitness Passport discounted gym benefitsPaid Parental leaveLong Service leave after 5 years40+ Locations across NSW and QLD if you choose to moveA values-based organisation that welcomes applicants from all faiths and backgrounds
