Work directly with the client, their families and friends, reporting to the Nurse Next Door Care Designer ensuring continuity of care.
Supporting every client in promoting happier ageing, and providing safe client care in accordance with the Australian Aged Care and NDIS standards and Nurse Next Door policies.Responsibilities:Creates quality care experiences and client focused care in homeDelivers on our Brand Promise by owning the client's experience, ensuring that you do whatever it takes to bring peace of mind to our clients and their familiesAssist clients with activities of daily living: healthy meal preparation, personal care, light housekeeping, companionship, manual handling, medication administration, as requiredMeets safety standards,



using equipment safely and accuratelyProvides premium care approach to clients and their family membersResponds to Care Services Centre (CSC) Team regarding scheduling inquiries and updates within the agreed timelinesReviews the care plan prior to all visits, completing records accurately and completing all tasks as requestedEngages in open communication with your Care Designer (RN) when opportunities to elevate your client's experience and care are identifiedUpdates availability and leave requests (as per stated expectations) with HR to ensure continuity of care for your clientKnowledge and understanding of standard infection control principlesClinical assessment and documentation skillsPrepares, administers, and documents all actions taken specific to prescribed oral, topical, subcutaneous and intramuscular medications.Observes, reports and records adverse reactions to medications and/or treatment to the Care Designer.Provides direct client care as defined under the scope of nursing practice and using sound judgment and critical thinking.Recognises the need for and implements emergency actions in appropriate manner, when indicated.Key Metrics:Attendance & ReliabilityUtilising AlayaCareNPS Client Score of 10Availability:Flexibility: weekend, public holiday, inactive overnight availability is a mustBusiness Relationships:Reports to the Clinical Operations ManagerWorks closely with Care TeamRequirements:AHPRA Registration or completed Cert lll in Aged Care / Disability or Individual SupportCommunity experienceMinimum 2 years of disability support community experience requiredTwo positive professional referencesPolice Check (Unsupervised Access)NDIS WC and current Blue CardAbility to travel independently to client's visitsCurrent driver's licence required for independent travel to client visitsExcellent communication skills, both written and verbalAbility to complete e-Learning
