Intake & Service Delivery Lead - Support at HomeAs the Support at Home Lead – Driving Intake & Service Delivery, you will work closely with the existing Support at Home Team to ensure clients experience a smooth transition from initial referral through to commencement and ongoing delivery of services.
You will provide hands‐on operational leadership, helping ensure services are coordinated effectively, client needs are understood and communicated, operational issues are addressed promptly and budgets and financial targets are supported and maintained.Key responsibilitiesCoordinating referrals, enquiries and client intake processesSupporting client onboarding and commencement of Support at Home servicesEnsuring client information, service requirements and documentation are accurate and completeDrafting agreements and tracking budgets and plan spendingsSupporting effective service scheduling and workforce coordinationMonitoring service delivery and responding to changes in client needsIdentifying and resolving service delivery issues before they impact clientsMaintaining accurate client and service recordsSupporting compliance with aged care requirements, organisational policies and service standardsMonitor service performance, capacity and operational prioritiesAbout youExperience within aged care or home care industry desirableExperience in client intake, service coordination, care coordination or operational service deliveryStrong organisational and prioritisation skillsThe ability to manage competing client and operational requirementsStrong communication and relationship-building skillsSound judgement and the ability to resolve day-to-day service delivery issuesStrong computer literacy and accurate record-keeping skillsAn understanding of quality, compliance and client-centred service deliveryThe ability to work collaboratively while also taking ownership of your responsibilitiesSalary depending on experienceFull-time employment based in ChildersOperational role with genuine responsibility and autonomySupport from an experienced management team
