It involves development of personalised care plans and budgets and liaison between members, families and health professionals to ensure high quality care.Key ResponsibilitiesCoordination of member services tailored to individual needs, delivered within budget and service expectationsAssist with member assessment and care plan developmentUse a person-centred approach with members, their families and healthcare professionalsAbout YouHold (or are working toward completion of) Certificate III in Individual Support, Health Services Assistant or equivalent or Cert IV in Aged Care,
