Case Manager/ Registered Nurse - KellyvilleThe Co-Group are hiring an experienced and compassionate Registered Nurse / Case Manager to join our growing team in Kellyville.
This is an excellent prospect for a dedicated healthcare professional who is passionate about supporting older Australians and people living with disability to achieve their goals and maintain independence.This flexible casual role offers approximately 28 hours per week across 4 days, with the option to work school-friendly hours for the right candidate.About the RoleAs a Registered Nurse / Case Manager, you will play a key role in assessing client needs, developing and reviewing care plans, coordinating supports, and providing ongoing clinical oversight.
You will work closely with clients, families, support workers, healthcare professionals, and community stakeholders to ensure high-quality, person-centred care.You will also contribute to business development activities, community engagement,



and the ongoing growth of our services.Key ResponsibilitiesConduct client assessments and develop person-centred care plansMonitor client outcomes and review support regularlyCoordinate referrals and liaise with service providers and stakeholdersProvide clinical guidance and oversight for clients requiring nursing supportMaintain accurate documentation and client recordsBuild strong relationships with clients, families, community organisations, and referral partnersSupport business growth through community networking and relationship managementContribute to quality improvement and clinical governance initiativesAbout YouTo be successful in this role, you will have:Current AHPRA registration as a Registered NurseMinimum 2 years' experience in Aged Care and/or Disability ServicesDemonstrated case management experienceStrong knowledge of person-centred care planning and NDIS or Home Care frameworksExperience in wound care, continence assessments, and clinical support services (highly regarded)Excellent communication and relationship-building skillsAbility to work autonomously while contributing to a collaborative team environmentCurrent Driver's Licence and access to a reliable vehicleCurrent NDIS Worker Screening Check, National Police Check, First Aid and relevant compliance requirements
