The company is committed to professional development, clinical supervision, and creating a supportive team workplace.POSITION OVERVIEWThe Community Aged Care Nurse delivers nursing care directly in clients' homes and community settings, supporting older clients to remain safe, well, and independent.
This is initially offered as a contract position, with the opportunity to transition to full-time employment in the future depending on caseload growth and operational needs.
The role involves building strong relationships with clients and families, coordinating with other allied health professionals, and adapting care delivery to each client's home environment.KEY RESPONSIBILITIESConduct nursing assessments and health monitoring in clients' homesDevelop and review individualised care plans in consultation with clients, families, and the broader care teamAdminister medications and clinical treatments as requiredProvide wound care, continence care,



and other clinical nursing tasks as neededIdentify and elevate changes in client health status promptlyMaintain accurate, timely clinical documentation for each visitSupport clients and families to understand and manage ongoing health needsPlan and manage a caseload of home visits across a designated areaQUALIFICATIONS & REGISTRATIONBachelor of Nursing (or equivalent)Current registration with AHPRA (Nursing and Midwifery Board of Australia)Working with Children CheckNDIS Worker Screening CheckCurrent Police CheckSKILLS & EXPERIENCEExperience in community, home-visiting, or aged care nursingConfidence working independently and managing a home-visit scheduleStrong interpersonal skills for building rapport with older clients and familiesGood time-management skills to coordinate visits across a geographic areaCurrent Australian driver's licence and access to a reliable vehicleWHAT AH2U OFFERSCompetitive contract ratePotential pathway to full-time employment as caseload buildsToll reimbursement and daily travel allowanceSupportive multidisciplinary team environment
