About the role

Join the dedicated team at Cooinda House, a specialised aged care service within Community and Oral Health, where you'll play an important role in supporting older people with complex healthcare needs. This position offers the opportunity to provide contemporary, evidence-based nursing care in a residential aged care environment that values compassion, dignity and person-centred practice.

Working collaboratively with an interprofessional healthcare team, you will deliver high-quality clinical care, respond to changing patient needs and support residents and their families throughout their healthcare journey. The role also contributes to clinical excellence through education, quality improvement activities and the promotion of best-practice nursing care. 【1-00d225】

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Apply clinical nursing expertise in assessment, planning, implementation and evaluation to deliver high-quality, person-centred care for residents of Cooinda House.
- Respond effectively to changes in resident health status through comprehensive assessment, clinical judgement and timely intervention.
- Collaborate with residents,



families and multidisciplinary healthcare teams to support positive health outcomes and quality of life.
- Support an evidence-based learning environment through participation in professional development, education and quality improvement activities.
- Contribute to a safe and supportive care environment through effective communication, teamwork and adherence to professional standards, policies and legislation.
