About the Role

As a Social Worker - Discharge Patient Flow Concierge, you will play a pivotal role in supporting patients throughout their healthcare journey, ensuring smooth and effective transitions from hospital to home or other care settings. Working as part of a multidisciplinary team, you will provide comprehensive psychosocial assessments, identify barriers to discharge and develop tailored intervention plans to support positive patient outcomes.

This dynamic role requires strong collaboration with patients, families, carers, community services and healthcare professionals to facilitate discharge planning and coordinate complex care arrangements. You will also lead and facilitate family meetings, advocate for patient needs and contribute to the delivery of high-quality, patient-centred care. With a focus on patient flow and continuity of care, you will use your clinical knowledge, organisational skills and problem-solving abilities to manage competing priorities in a rapid-paced healthcare environment.

About You

You are a compassionate and proactive Social Worker who is passionate about supporting patients and their families through complex and often challenging circumstances. Holding a Social Work qualification that provides eligibility for membership with the Australian Association of Social Workers (AASW), you have a strong understanding of the role of Social Work within a health setting and a commitment to achieving the best possible outcomes for patients.

You are skilled in conducting psychosocial assessments, identifying patient needs and developing appropriate intervention strategies to support safe and effective discharge planning.



Your collaborative approach enables you to build strong working relationships with patients, families, carers and multidisciplinary teams, while your ability to organise and facilitate family meetings ensures all stakeholders are engaged in the care planning process.

With well-developed communication, written and computer skills, you can confidently manage complex caseloads, balance competing demands and coordinate comprehensive case management plans. You thrive in a team environment, are highly organised and bring a patient-focused approach to everything you do.

What you'll be doing
To provide an effective and efficient Social Work service that addresses the psychosocial aspects of care for patient, as identified.
To provide intervention including support, education and resources to people in the context of their bio-psycho-social situation, to assist in dealing with health events.

The Discharge Patient Flow Concierge Social Worker is responsible for:

- Supporting and facilitating discharge across MNCLHD clinical networks.
- Assisting patients and families to transition from hospital to home or another care setting.
- Proactively identifying patients at risk of delayed discharge because of complex medical, social, or logistical issues.
- Working with multidisciplinary discharge-planning teams.
- Providing social work interventions, including support, education, and resources, within the context of the person’s biopsychosocial circumstances.
- Working alongside a Discharge Patient Flow Concierge Administration Officer.




- Addressing psychosocial factors and barriers affecting discharge.

This position is a network role embedded within the Social Work Department, with access to professional mentoring, supervision, and departmental support.
