About the Role

CatholicCare Central Queensland is seeking an experienced and values-driven Community Care Manager to provide operational leadership and management of our community aged care services within an allocated geographical area.

This is an opportunity to lead multidisciplinary teams and make a meaningful difference in the lives of older people, supporting them to remain independent, connected and living well in their own communities.

You will be responsible for achieving strong client, workforce, financial, quality and operational outcomes while ensuring services are safe, responsive, person-centred and compliant with legislative, contractual and regulatory requirements.

Working collaboratively with other Community Care leaders across the Diocese, you will play an important role in strengthening service delivery, supporting aged care reform and contributing to the sustainable growth and continuous improvement of CatholicCare's community aged care services.

Key Responsibilities

- Provide operational leadership and management of community aged care services within the allocated region.
- Lead, coach and develop multidisciplinary teams to achieve strong workforce and client outcomes.
- Ensure the delivery of safe, high-quality and person-centred services that promote client choice, independence and wellbeing.
- Manage budgets, workforce utilisation, resources and operational performance to support financial sustainability.




- Maintain strong governance, quality, safeguarding and risk management practices.
- Ensure compliance with legislative, contractual, accreditation and organisational requirements.
- Build productive relationships with clients, families, employees, health professionals, referral partners and community organisations.
- Support the implementation of aged care reform, organisational initiatives and service improvements.
- Identify opportunities to strengthen service delivery, community partnerships and sustainable growth.

Qualifications and Experience

Essential:

- Relevant tertiary qualification in business, management, community services, health or a related discipline, or an equivalent combination of qualifications and demonstrated experience;
- Experience in the delivery of Commonwealth Home Support Program, Support at Home or other government funded community aged care services; and
- Current C class Driver’s license (QLD)

Desirable:

- Postgraduate qualification in leadership, management, business or a related discipline.

Core Competencies

To be successful in this role, you will demonstrate:

- Solid operational leadership capability, including the ability to lead, coach and develop multidisciplinary teams, manage operational performance and build workforce capability.
- Strong knowledge of community aged care, including Home Care Packages,



Commonwealth Home Support Programme, Support at Home and other community-based aged care services.
- High-level financial and business management capability, including experience managing budgets, workforce utilisation, operational performance and resource allocation.
- Solid governance, quality and risk management capability, including the ability to ensure compliance and drive continuous improvement.
- High-level stakeholder engagement and communication skills, with the ability to build productive relationships with clients, carers, employees, health professionals, referral partners, community organisations and government stakeholders.
- The ability to positively represent CatholicCare and strengthen relationships and partnerships within the community.

Other Requirements

Ability and willingness to undertake regular travel throughout the Diocese of Rockhampton.
