Stream Services is seeking compassionate and experienced Lifestyle Support Workers to join our growing team in Clermont, QLD . This rewarding role supports people with disability to live independently, stay connected with their community, and achieve meaningful personal goals.
What You’ll Do
- Support NDIS Participants with daily living activities, including personal care, meal preparation and household tasks
- Provide social and recreational support to build confidence, independence and community participation.
- Develop and follow individualised support plans aligned with Participant goals
- Maintain accurate NDIS compliant documentation and reporting.
- Work collaboratively with the wider team to deliver person-centred, high-quality care.
About You
We’d love to hear from you if you have:
- Qualifications in disability support, aged care, community services – or equivalent experience as a carer.
- Experience providing person-centred support to NDIS or aged care clients (preferred but not essential).
- A warm, empathetic and professional approach when supporting others.
- Strong communication skills, and the ability to build positive relationships.

- Commitment to upholding the rights and dignity of people with disability
- Ability to work independently and as part of a supportive team
- Current First Aid and CPR Certificates (or willingness to obtain)
- NDIS Worker Screening Check and Blue Card (or willingness to obtain)
- Valid driver’s licence.
