Phoenix 360 Degree Service is seeking an experienced Residential Care Officer to provide person-centred care and structured support to people with disability living in SIL and other group settings. You will work closely with participants, families, guardians and health professionals to understand needs and deliver tailored support plans.

The role involves assessment, planning, documentation and progress reporting to promote independence, safety and quality of life, with some rostered shifts
