NSW Health, Concord Repatriation General Hospital, is seeking a Clinical Aged Care Assessor (Social Worker/Physiotherapist/Occupational Therapist Level 1/2 or Registered Nurse) for a temporary full time role. The position involves triaging referrals, conducting comprehensive assessments and coordinating care for frail older people, under the Aged Care Act 2024.

The ideal candidate will have strong communication skills, ability to screen referrals, and hold a valid NSW driver’s licence.
