South West Hospital and Health Service in Roma, QLD is seeking a Senior Aged Care Assessor to perform high-level clinical and non-clinical aged care assessments for frail older people and carers, in line with Commonwealth Health guidelines. The role focuses on recommending service options and assisting access to appropriate care.

The Assessor will establish eligibility for Commonwealth Aged Care Programs, with outreach in Roma and surrounding facilities.
