About the role:

An opportunity exists for an enthusiasticPhysiotherapist with Neurologicalskills to join our collaborative and dynamicRehab at Hometeam.Rehab at Homeoperateswithin the Division of Western Health at Home. Rehab at Home is a subacute, bed substitution program thatprovides intensive, short-term rehabilitation to patientsdischarged early from hospital.

Key responsibilities

- Provision of intensive rehabilitation for clients who have been discharged early from hospital to sustain discharge and prevent representation to hospital.
- Provision of goal-directed therapy in clients’ homes, in the centre, or via telehealth (Health Direct).
- Collaborating with the interdisciplinary team to achieve client directed goals.

About you

- Experience in assessing and managing clients with a variety of neurological conditions.
- Excellent organisation and time management skills.
- Experience supervising Grade 1 Physiotherapists, Physiotherapy students and/or Allied Health Assistants.
- Demonstrated experience in quality improvement and/or research.
