NSW Health is seeking a Youth Health Worker to join the High Street Youth Health Service and Western Area Adolescent Team. The role focuses on youth-focused care, case management and health education, supporting vulnerable young people aged 12–24 in a diverse, multidisciplinary setting.
You will provide responsive, evidence-based services to improve access to healthcare and advocate for young people, developing individual care plans and coordinating care within a primary healthcare framework.
