Yellow Bridge is seeking a reliable Support Coordinator to join our team in Canberra, ACT. You will coordinate formal and informal supports, manage complex needs, and liaise with mental health, housing, and health sectors to promote community participation.

We require a Certificate IV in Community Services (or related field) and a minimum of two years' experience with complex mental health needs. A driver's license and Working with Vulnerable People registration are essential.
