About the Role
As a Support Coordinator, you will support neurodivergent NDIS participants to understand, implement and maximise their NDIS plans while building their capacity, independence and confidence.
Working alongside our Senior Support Coordinator and leadership team, you'll help participants connect with services, build meaningful support networks, overcome barriers, and achieve their goals.
Key Responsibilities
- Coordinate and implement NDIS supports aligned with participant goals.
- Assist participants to understand and utilise their NDIS funding effectively.
- Connect individuals with appropriate community, mainstream and funded supports.
- Develop and maintain strong relationships with participants, families, carers and service providers.
- Monitor participant progress and prepare for plan reviews.
- Support participants through life transitions and changing circumstances.
- Advocate for participant choice, control, rights and self-determination.
- Work collaboratively with multidisciplinary teams and external stakeholders.
- Maintain accurate documentation, case notes and compliance requirements.
About You
You are passionate about supporting neurodivergent individuals and bring a strong understanding of the NDIS and disability sector.
Essential Requirements
- Minimum 12 months' experience as a Support Coordinator or in a similar case management role
- Demonstrated experience working with neurodivergent individuals, particularly autistic people
- Strong knowledge of the NDIS, including legislation, funding, policies and service systems
- Excellent communication, relationship-building and active listening skills
- Ability to adapt your communication style to individual needs
- Strong organisational, time management and administrative skills
- Current First Aid & CPR Certificate
- Current Queensland Working with Children Check (Blue Card)
- Current Driver's Licence and reliable vehicle
- Full working rights in Australia
Desirable
- Diploma or higher qualification in Community Services, Disability Support or a related field
- Yellow Card
- NDIS Worker Orientation Module completion
