Job Description
Five Positive Friends is growing its Care Team in South Brisbane to coordinate assessments, planning and delivery of care for Members at home. You'll work with Helpers and trusted partners, updating Help Plans and arranging allied health referrals with a focus on meaningful, person-centred support.

You'll collaborate with Care Specialists, Community Nurses and Workforce Coordinators to tailor solutions, using technology to streamline workflows and improve outcomes for Members and their families.
