About the Role

n

We are seeking compassionate, reliable and experienced Support Workers to provide high-quality care and support to NDIS participants located in Gympie.

n

This role involves supporting participants with moderate to high support needs in a home and community setting. We are looking for team members who are patient, respectful and committed to promoting dignity, independence and quality of life.

n

You will provide person-centered care support tailored to the participants individual needs, goals and routines. Duties may include:

n

- Personal care and daily living assistance
n
- Manual handling and mobility support
n
- Assistance with meal preparation and medication prompts
n
- Emotional support and companionship
n
- Community access and transport to appointments
n
- Encouragement to engage in community activities and social participation
n
- Skill development and independence mentoring
n
- Following support plans and care routines
n
- Maintaining accurate progress notes and documentation
n

n

What we're looking for:

n

n




- Experience supporting participants with moderate to high support needs, complex care, behaviors of concern or mental health support
n
- Willing and competent to undertake personal care responsibilities
n
- A genuine passion for supporting people with a disability
n
- Strong communication and interpersonal skills
n
- Ability to work independently and as part of a team
n
- Availability to work longer shifts, including weekends (if required)
n

Essential qualifications and licences:

n

- Certificate III in Individual Support or equivalent
n
- National Police Certificate
n
- NDIS Worker Screening Card
n
- Working with Children Card
n
- Drivers' licence and reliable insured vehicle
n
