The Cairns and Hinterland Transition Care Team delivers services across the Cairns, Cassowary, and Tablelands regions. While the majority of team members are based in Cairns, Transition Care Program (TCP) offices are also located at Mareeba Hospital and Gordonvale Hospital, where residential Transition Care services are provided.
The team delivers high-quality, low-intensity allied health, nursing, and case management services to both community-based and residential Transition Care clients following discharge from acute and sub-acute hospital settings. Clients are assessed by the Aged Care Assessment Team (ACAT) and deemed eligible for Transition Care, demonstrating the potential to remain living in the community with appropriate support, delay entry into residential aged care, or achieve a lower level of ongoing care through participation in the program. Join the team and.. * Provide comprehensive education on a defined range of topics to patients or groups of patients. * Actively contribute as a member of a multidisciplinary team, including where appropriate, leading departmental and team meetings, case conferences and other team projects and activities. * Initiate, plan, and evaluate quality improvement activities under the guidance of an allied health professional. * Provide support,
