Heart-led Support
Queensland
Job Description
Heart-led Support doesn't look like the rest of the sector.
n
We're built around male-to-male support. Our participants are men, and the supports we deliver are designed around what that actually takes. Presence. Straight talk. People who show up and keep showing up.
n
We work hard. We don't hide behind process. And the impact is real, you'll see it in the people you're responsible for.
n
This role is about half at a desk and half out in the community. You need to be good at both.
n
If that's your kind of thing, keep reading.
n
Who We Are
n
Heart-led Support is SEQ's fastest-growing team of elite support workers and leaders and we're raising the bar on what disability support actually looks like.
n
No box-ticking. No bureaucratic noise. Just real, skilled, grounded people doing meaningful work every single day.
n
We've built our structure deliberately. Frontline support, coordination, onboarding and operations all sit in their own lane, so our people can stay close to participants instead of drowning in admin.
n
That's where you come in.
n
The Role: Community Coordinator
n
You're the connection point between our participants, their families, our Disability Support Workers and the Operations Manager.
n
You make sure supports are working in practice, not just on paper.
n
Some days that's phone calls, records, rosters and follow-up from the office. Other days it's a participant's lounge room or a community outing. You hold the relationships, you back our DSWs, you chase the details, and you flag things early.
n
What You'll Be Doing:
n
Regular check-ins with participants, families and stakeholders, in person and in the community
n
Day-to-day supervision, coaching and practical feedback for our DSWs
n
Watching service coverage and working roster gaps before continuity is affected
n
Matching the right worker to the right participant
n
Checking documentation, notes and care plan records actually stack up
n
Following up quality gaps, low-level complaints and routine service issues
n
Spotting risk, safeguarding and pattern concerns and escalating them early
n
Keeping the Operations Manager across what's really happening on the ground
n
You Must Have;
n
Demonstrated experience in disability, community services, participant coordination or frontline leadership
n
Working knowledge of NDIS service delivery, the Practice Standards and the Code of Conduct
n
Experience supporting frontline workers with guidance, feedback and follow-through
n
Confidence with participant plans, risk information, routines and stakeholder communication
n
Clear, factual writing. Notes, records and management updates that hold up
n
Current NDIS Worker Screening Check, driver licence and reliable vehicle
n
Cert IV or Diploma in Community Services, Disability or Individual Support preferred. Real experience counts
n
You'll Thrive Here If;
n
You follow through. When you say you'll chase something, it gets chased.
n
You're calm when it's messy. Participant distress, a complaint, a roster falling apart at 6am. You don't flap.
n
You've got judgement. You'll sort routine stuff yourself without needing permission for every step,
and you know exactly when to stop and escalator instead of winging it.
n
You can sit with the admin. Records, notes and follow-up are half this job and they're the half that protects people. You do them properly.
n
You've spent years in this field not because it pays well, but because something in you is called to it.
n
You don't build workarounds. If something's broken you say so, on the record.
n
Bonus points if you carry experience in Men's Work, Martial Arts, Military, Security, Sport, or Coaching. These aren't just hobbies, they're frameworks for how you work.
n
What You'll Get;
n
Permanent Employment - Security, flexibility, and a pay-cheque that actually reflects your skill and commitment. Paid at SCHADS Level 4.
n
17.5% Annual Leave Loading - You get paid properly when you take a break. Because you've earned it.
n
Unmatched Team Culture - You'll be surrounded by people who take their craft seriously. No passengers. No dead weight. There is an incredibly huge amount of support around you.
n
No Micromanagement - You'll be trusted to run your patch your way, backed by a leadership team who's been in the field themselves.
n
A Clear Role - You'll know exactly what sits with you and what doesn't. No silent scope creep, no absorbing someone else's job.
n
Self-Development & Adventure Days - Workshops, retreats, and experiences designed to push you mentally, physically, and emotionally. We invest in the whole person.
n
Real Career Movement - We're growing quick and we promote from within.
n
This role will stretch you
n
It will demand your full presence, your judgement, and your genuine passion for the work. Our participants deserve someone who's paying attention.
