Job Description

Focus Care is seeking a Care Manager/Care Partner to support Home Care participants with effective care management in a rapid-paced setting. You will help participants access resources to meet identified goals within funding and quality constraints.

The role involves participant screening, onboarding, non-clinical assessments, risk mitigation, planning and implementing support strategies, and transitioning to other services as needs change. Excellent verbal and written communication is essential.
