Heart-led Support is SEQ's fastest-growing team of elite support workers and leaders and we're raising the bar on what disability support actually looks like.

We've built our structure deliberately. Frontline support, coordination, onboarding and operations all sit in their own lane, so our people can stay close to participants instead of drowning in admin.
That's where you come in.
The Role: Community Coordinator
You're the connection point between our participants, their families, our Disability Support Workers and the Operations Manager.
You make sure supports are working in practice, not just on paper.
Some days that's phone calls, records, rosters and follow-up from the office. Other days it's a participant's lounge room or a community outing. You hold the relationships, you back our DSWs, you chase the details, and you flag things early.
What You'll Be Doing:
Regular check-ins with participants, families and stakeholders, in person and in the community
Day-to-day supervision, coaching and practical feedback for our DSWs
Watching service coverage and working roster gaps before continuity is affected
Matching the right worker to the right participant
Checking documentation, notes and care plan records actually stack up
Following up quality gaps, low-level complaints and routine service issues
Spotting risk, safeguarding and pattern concerns and escalating them early
Keeping the Operations Manager across what's really happening on the ground
You Must Have;
Demonstrated experience in disability, community services, participant coordination or frontline leadership
Working knowledge of NDIS service delivery, the Practice Standards and the Code of Conduct
Experience supporting frontline workers with guidance, feedback and follow-through
Confidence with participant plans, risk information, routines and stakeholder communication
Transparent, factual writing. Notes, records and management updates that hold up
Current NDIS Worker Screening Check, driver licence and reliable vehicle
Cert IV or Diploma in Community Services, Disability or Individual Support preferred. Real experience counts

You're calm when it's messy. Participant distress, a complaint, a roster falling apart at 6am. You don't flap.
You've got judgement. You'll sort routine stuff yourself without needing permission for every step,
and you know exactly when to stop and escalator instead of winging it.
You can sit with the admin. Records, notes and follow-up are half this job and they're the half that protects people. You do them properly.
You've spent years in this field not because it pays well, but because something in you is called to it.
You don't build workarounds. If something's broken you say so, on the record.
Bonus points if you carry experience in Men's Work, Martial Arts, Military, Security, Sport, or Coaching. These aren't just hobbies, they're frameworks for how you work.
