Brain Injury Care and Supports is a registered NDIS provider delivering high‑quality, person‑centered support coordination to participants living with Acquired Brain Injury (ABI), Autism, ADHD, Psychosocial Disabilities, Mental Health Conditions, Physical Disabilities, and complex support needs.

Position Overview





As a Level 3 Support Coordinator, you will support NDIS participants with complex needs by coordinating services, strengthening capacity, and ensuring supports are delivered safely and effectively. You will work across all support environments, including in‑home supports, community access, Supported Independent Living (SIL), and complex care arrangements.

A key part of this role involves assisting participants with Supported Independent Living (SIL) arrangements. You will help participants identify suitable SIL options, understand service choices, and transition smoothly into SIL environments. You will monitor service quality, advocate for participant rights, and support changes when issues arise- ensuring participants have a safe, comfortable, and personalized living setting that adapts to their needs over time.

Key Responsibilities

Participant Coordination & Support

Assist participants to understand and utilize their NDIS plans

Coordinate Supported Independent Living (SIL) supports

Strengthen participant capacity to manage their own supports

Support participants with ABI, psychosocial disability, physical disability, and complex presentations

Work across in‑home supports, community access, SIL, and complex care environments

Stakeholder & Relationship Management

Build collaborative, trust‑based relationships with participants, families, and providers

Attend home visits, community appointments, and family meetings

Represent the organization at events, expos, and networking opportunities

Engage with mainstream services and informal supports

Documentation & Compliance

Maintain accurate records and participant documentation

Prepare reports, progress updates, and NDIS‑required submissions

Work collaboratively with internal clinical and operational teams

Qualifications & Experience

Required

Minimum 2 years’ experience as a Support Coordinator (Level 2 or 3), Psychosocial Recovery Coach, or Specialist/Complex Coordinator

Demonstrated experience coordinating SIL supports

Strong understanding of NDIS legislation, reporting, and compliance

Experience supporting participants with psychosocial disability, ABI, or physical disability

Preferred

Certificate IV in Mental Health, Disability, Community Services, Peer Work, or relevant tertiary qualification

Trauma‑informed or recovery‑oriented practice training

Mandatory

Current Driver’s License, registration, and insurance

NDIS Worker Screening Check

Blue Card (or ability to obtain)

First Aid & CPR certification

Skills & Attributes

Strong NDIS knowledge and ability to navigate complex plans

Excellent communication, advocacy, and interpersonal skills

A genuine enjoyment of building relationships and solving problems

High level of cultural competence and empathy

Confident supporting participants experiencing mental health challenges

Proficient in Microsoft Office and electronic record systems

Comfortable travelling across Brisbane and surrounds
