JOB DESCRIPTION

You'll support adults with chronic and complex health needs to build independence and improve their quality of life through practical, person-centred care.
n
You'll partner with clients, carers and other health professionals to deliver evidence-based services in home and community settings. This includes:
n
n
- Completing comprehensive assessments, care planning and home visits
n
- Recommending assistive technology, equipment and home modifications
n
- Supporting clients to navigate aged care and community funding pathways
n
- Delivering education, coaching and self-management strategies
n
- Collaborating with a multidisciplinary allied health team
n
- Participating in supervision, case reviews and continuous improvement initiatives
n
- Maintaining high-quality documentation and contribute to service outcomes
n

- Current AHPRA registration

- Experience or knowledge in functional assessments, home modifications and assistive technology

- Strong communication, organisation and problem-solving skills

- The ability to work effectively with people from diverse cultural and linguistic backgrounds

- A commitment to evidence-based practice and high-quality client care
