About the Role
n Concord Repatriation General Hospital has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.

n

As a member of a multidisciplinary Aged Care Assessment Service, an assessor triages referrals via the My Aged Care Assessor portal and conducts comprehensive aged care assessments, provide care planning and care coordination for frail older people, while working within the legislative requirements of the Commonwealth Aged Care Act 2024.

Ideal Candidate

n

n
- Well developed written and verbal communication skills including demonstrated computer literacy.
n
- An understanding of how to screen and triage referrals as well as set priorities based on priority criteria.
n
- Current NSW Drivers Licence (P1 and P2 acceptable).
n
