APM is seeking a Clinical Aged Care Assessor who will identify needs of older Australians and connect them with services to support independent living. The role requires daily travel across the service area and collaboration with families and a multidisciplinary team.

You will perform thorough assessments, coordinate referrals, and promote functional independence while adhering to health department requirements. Training and development are provided to support success in the role.
