What you'll do:

- Deliver high-quality, person-centred nursing care in accordance with organisational policies, ANMC standards, and professional codes

- Conduct holistic assessments in home or community settings and develop individualised care plans to promote independence and wellbeing

- Coordinate, implement, and evaluate care plans to achieve optimal client health outcomes in the community

- Work collaboratively with multidisciplinary teams, external providers,



and community services to support client needs

- Monitor client health status and escalate concerns or changes in condition appropriately

- Maintain accurate, timely, and compliant clinical documentation in line with organisational and regulatory requirements

- Practise safely within scope, ensuring accountability and adherence to policies, procedures, and community care standards

- Maintain up-to-date clinical knowledge and skills relevant to community-based care through ongoing career development

- Provide leadership, supervision, and support to Enrolled Nurses, support workers, and care staff

- Assist with staff orientation, education, and ongoing competency development

- Support student placements and assist with onboarding of agency or new staff

- Contribute to continuous improvement, service development, and quality care initiatives

- Communicate effectively with clients, families, carers, and stakeholders to support coordinated care

- Promote client independence, health education, and self-management strategies

- Escalate incidents, risks, and safeguarding concerns in line with reporting requirements

- Act as a senior clinician in the community setting when required, including responding to urgent situations

- Ensure compliance with quality standards, accreditation requirements, and Work Health & Safety practices in community environments





- Identify and manage risks within clients’ homes and community settings to maintain safe care delivery

- Undertake additional duties within scope to support effective and responsive community care services.

Essential:

- Current AHPRA registration with no conditions

- National Police Clearance

- Current NDIS Worker Screening Check

- First Aid Certificate / CPR

- Excellent time management skills

- Ability to work efficiently under pressure.
