Intereach is looking for aCommunity Support Worker (Home Care)to join our Aged Care Support team inAlbury. This is apermanent part time position (minimum15 hours per week)based onsite, supporting older people tomaintaintheir
independence,wellbeingand
connection to their community.
n
In this rewarding role,you'llprovide person-centred support to people aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people), helping them continue living safely and comfortably in their own homes.
n
What You'll Be Doing
n
As a Community Support Worker,you'llprovidea range of services tailored to each client's needs, including:
n
n
Respite care and companionship
n
Domesticassistanceand
household support
n
Social support and community engagement
n
Support with recreational and social activities
n
Encouraging independence through a reablement and wellness approach
n
Reporting on client wellbeing and progress
n
Maintainingaccurateclient
records and documentation
n
n
Your work will empower older people to remain
active,connectedand
independent while receiving the support they need to continue living at home.
n
Who We're Looking For
n
We'dlove to hear from you if you:
n
n
Hold a minimumCertificate III in Aged Care, Disability,Homeand Community Care or equivalent
n
Have experience providing personal care and




domesticassistance,
or a strong interest in supporting older people
n
Have a sound understanding of the needs of older people,carersand people with disability
n
Can work effectively both independently and as part of a team
n
Demonstratestrong communicationand customer service skills
n
Are committed to a person-centred, strengths-based approach to care
n
Have a current unrestricted Australian Driver Licence and access to a fully insured vehicle
n
Hold current First Aid and CPR qualifications
n
Are comfortable using a smartphone and basic Microsoft Office applications
n
n
Mandatory Requirements
n
To be successful in this role, you must be able to obtain or hold:
n

Current Australian Driver Licence
n
Criminal Record Check
n
Aged Care clearance and eligibility to work in aged care
n
Current First Aid and CPR Certificates
n
Smartphone for work-related communication and documentation
n
n
