About the role:
An opportunity exists for an enthusiasticPhysiotherapist with Neurologicalskills to join our collaborative and dynamicRehab at Hometeam.Rehab at Homeoperateswithin the Division of Western Health at Home.
Rehab at Home is a subacute, bed substitution program thatprovides intensive, short-term rehabilitation to patientsdischarged early from hospital.
Key responsibilities
Provision of intensive rehabilitation for clients who have been discharged early from hospital to sustain discharge and prevent representation to hospital.
Provision of goal-directed therapy in clients' homes, in the centre, or via telehealth (Health Direct).
Collaborating with the interdisciplinary team to achieve client directed goals.
About you
Experience in assessing and managing clients with a variety of neurological conditions.
Excellent organisation and time management skills.
Experience supervising Grade 1 Physiotherapists, Physiotherapy students and/or Allied Health Assistants.
Demonstrated experience in quality improvement and/or research.
