Adult Social Worker - Transfer of Care South Team
Are you a qualified Social Worker seeking a rewarding hospital-based role with scope for autonomy and professional growth?
Join our South Transfer of Care Team, covering Addenbrooke's Hospital, other local surrounding hospitals and Community Rehabilitation Units.
You will work with adults aged 18+ to support safe discharges, arrange care and support, and undertake the six-week post-discharge review to ensure ongoing needs are met and outcomes are sustained.

What will you be doing?
You will undertake Care Act Assessments for adults to facilitate safe discharge from hospital and community rehabilitation services, identifying care and support needs and arranging appropriate services to promote independence and wellbeing.
You will also undertake safeguarding enquiries and support individuals where there are concerns relating to risk, abuse or neglect.
As part of the South Transfer of Care Team, you will complete six-week reviews following discharge, evaluating the effectiveness of support arrangements and making adjustments where required to ensure outcomes continue to be met.
Working within a multidisciplinary environment, you will collaborate closely with therapists, nursing staff, health colleagues, care providers and community services to deliver person-centred, strengths-based support.
You will communicate effectively with individuals, their families and professionals, ensuring decisions, assessments and outcomes are clearly recorded.
This is a fast-paced and rewarding role where you will play a key part in supporting people to leave hospital safely, regain independence and achieve the best possible outcomes.
The Transfer of Care Teams operate within office hours, Monday to Friday,
and we will consider flexible working arrangements.
Our team provides strong peer and managerial support, regular supervision and excellent opportunities for professional development.
Interviews are usually held face-to-face and may include an assessment, although full details will be provided in advance.
