The ATSICHS Vulnerable Youth Service team provides tailored case management support for young Aboriginal and Torres Strait Islander peoples.
The team incorporates various services and funding streams, all focused on vulnerable young people.

The 'Vulnerable Youth (Youth Justice, CYRD)' role is funded through the Community Youth Response Diversionary (CYRD) Intensive Case Management Program.
The role supports high risk/complex needs young people, at risk of re-offending or entering the Youth Justice system.
This is achieved by a strong assertive outreach focus and delivering tailored holistic supports to young people.
The model incorporates a family connection approach for delivering targeted supports and interventions, working also with immediate/extended family to support the young person.
Duties
Effectively engage clients through cold-call home-based visits.




Turn initial 'service offerings'home visitintoconsentby delivering motivational interviewingandpositive reinforcementpractice interventions.
Deliver immediate safetyassessment and safetyresponses, documented and approved by the line manager,in instances of domestic and family violence, self-harm/mental health,andchildabuse.
Facilitate ATSICHS 'Narrative Assessment Tools' completed in fullby client ensuring the voices of clients are imbedded in their assessment.
Facilitate conversations and introspection onall assessment domains.
Deliver Empowerment Interventions that Build Client Belief, Motivation,and Self-Sufficiency.
Deliver case managed services according to the program model of care, ensuring minimum case load requirements aremaintainedto ensureequitableaccess to services for community.
Maintainaccurateand detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
CompleteSRS (client-ledbrief session rating survey) upon each occasion of direct face-to-face support provided to client.
As perMOBnetPerformance module,participatein monthly prescheduledsupervision with direct lineManager.Ensure Supervision tasks are thepriorityin work completion.Ensure all associated monthly supervision tasks inMOBnetarecompleted (two-way feedback, team culture score, etc.).

Base Salary of $85,214 per annum (Based on qualification and experience).




12% Super + access to salary packaging (Increases your take home pay).
Permanent Full-Time position.

Desired Skills and Experience
Required
Limited/No Qualifications + Moderate/Low Work History: Less than 8 years working full-time in another industry and a commitment to complete Diploma (funded by ATSICHS) while working.
OR
Limited/No Qualifications + Strong Work History with Transferrable Skills: At least 8 years working full-time in another industry and a commitment to complete Diploma (funded by ATSICHS) while working.
AND
Soundcase management skills,includinggrammaticalwritingability, personal organisationand time management,and execution of services according to themodel of care phases of work.
Demonstratedability tounderstand and applysocial worktheoreticalframeworks ofpractice, aligned with ATSICHSvalues.
Ability todeliverother staff onclient interventionsthatbuildclientmotivation,insight andacceptance,self-awareness,decision making (agency),andsustainable behaviour change.
Ability toapplytechnicalknowledgeof relevant legislation(child protection/housing/youth justice etc).
Ability tocritique andproblem solvelegislativegreyzones, while managingorganisational and child/clientharm risks.
Ability toproactively engage and developstrong working relationshipswithdecisions ofexternal stakeholders, includingChild Safety,Youth Justice,Schools, andHospitals.
Ability toidentify through systems panel coordinationnumerousinternal ATSICHS programs and services toactivatewrap-around care forclients,trading-off and negotiating resources across theorganisationand between other teamsto achieveclient impact.
Additional Requirements
National Police Certificate.
Current Working with Children's Card (Blue Card) or be eligible toapply.
Driver's Licence.
Evidence of Work Rights.
Travel to work in the community on a regular basis.
