Clinical Support Worker (Adult Community)
Integrated Care Team The Clinical Support Worker role within the Integrated Care Team is an integral part of the service, providing patient centred care to the residents of Wakefield and the five towns area within their preferred place of care.
Key responsibilities Deliver holistic, protected, and effective care in line with Trust values and behaviours.
Support nurses and other healthcare professionals in providing patient care.
Provide personal care to Palliative Care and Rehabilitation (Rehab) patients in their own homes, including: Bathing Toileting Dressing Repositioning patients Assisting with personal hygiene and appearance Working requirements The Integrated Care Team operates 24 hours a day, 7 days a week.
You must be willing and able to work:



Days and nights Weekends and bank holidays as required The role involves significant travel between patients' homes.
