Danila Dilba Health Service is seeking a Transient and Outreach Support Coordinator to assist clients experiencing homelessness and other complex health and social issues.
You will help clients navigate health and community services and connect them with culturally appropriate supports.
The role requires 3+ years in primary health care (or equivalent), knowledge of social determinants of health, and robust communication with diverse stakeholders.
