Launch Housing in Melbourne's inner north is seeking a Northern Outreach Case Manager to support people experiencing homelessness or at risk, using an assertive outreach approach across City of Yarra, Merri-Bek and Darebin.
Based at Collingwood, you will build therapeutic relationships and develop recovery-focused case plans.
You will work in clients' homes or on the street, help with housing, income entitlements, employment and health services, and maintain accurate records while collaborating
