The Royal Melbourne Hospital is seeking an experienced and innovativeNurse Practitioner - Delirium and Cognitive Careto provide advanced clinical leadership, expert assessment and evidence-based care for patients experiencing delirium, dementia and other cognitive impairments across inpatient services.
n
Working collaboratively within multidisciplinary teams, the Nurse Practitioner will drive best-practice care, support early recognition and management of delirium, enhance patient and carer experiences, and contribute to service development, education, research and quality improvement initiatives.
n
This is an exciting opportunity for an experienced clinician to influence clinical practice across RMH and lead initiatives that improve outcomes for some of our most vulnerable patients.

Provide advanced clinical assessment, diagnosis, management and follow-up for patients with delirium, dementia and cognitive impairment.
n
Lead evidence-based delirium prevention, assessment and management, including quality improvement to reduce hospital-acquired delirium.
n
Provide expert clinical leadership, consultation and education to multidisciplinary teams.
n
Coordinate referrals, care planning, medication management, discharge planning and transitions for patients with complex cognitive health needs.
n
Lead and contribute to research, audits, evaluation, policy development and professional education initiatives.
n
Registered Nurse and endorsed Nurse Practitioner in Australia, with a Master of Nursing and current MBS/PBS prescriber credentials.
n
Advanced clinical expertise in delirium and cognitive care, with autonomous practice and strong clinical reasoning.
n
Demonstrated clinical leadership, including mentoring, education and multidisciplinary collaboration.
n
Highly developed communication, stakeholder engagement and problem-solving skills.
n
Commitment to quality improvement, research, professional development and safe, person-centred care, with proficiency in EPIC EMR.
