Wellways is seeking a Health Linker to help people access health, community and social supports to improve wellbeing and recovery goals. This is a people‐focussed role that looks beyond appointments to meaningful connections and participation.

You will work with GPs, health professionals and community organisations to build coordinated pathways, reduce barriers and support individuals to navigate services, with a commitment to recovery‐oriented, trauma‐informed, and person‐led practice.
