Carle Health is seeking an Outpatient Mental Health Clinician to join our Healthcare & Medical Services department and support our mission of improving patient health outcomes through high-quality, community-based care.
In this role, you will provide outpatient mental health services to a diverse panel of patients, delivering evidence-based therapeutic interventions such as individual, group, and family therapy.
You will conduct comprehensive biopsychosocial assessments, develop and regularly update person-centered treatment plans, and collaborate closely with psychiatrists, primary care providers, nurses, and other members of the interdisciplinary care team to ensure coordinated, safe, and effective care.
The Outpatient Mental Health Clinician will perform ongoing risk assessments, including suicide and violence risk screenings, implement appropriate safety planning, and respond to mental health crises using de-escalation techniques and established organizational protocols.
You will document all clinical encounters in the electronic health record in a timely, accurate, and compliant manner, ensuring adherence to Carle Health policies as well as state and federal regulations, including HIPAA and other privacy requirements.
You will also support care transitions by coordinating referrals to community resources, higher levels of care, and social services as appropriate to patient needs.
Clinicians in this role are expected to maintain a caseload that balances direct patient care, documentation, and care coordination.
You will monitor treatment progress using standardized outcome measures where appropriate, adjust interventions based on clinical response, and actively engage patients and families in shared decision-making.
As part of Carle Health's commitment to quality and innovation, you will participate in case consultations, team huddles, and quality improvement initiatives designed to enhance patient safety, access, and outcomes in outpatient behavioral health.

The Outpatient Mental Health Clinician will participate in regular supervision or peer consultation, complete required continuing education, and stay current with developments in evidence-based mental health treatment.

The role requires strong clinical judgment, excellent communication skills, and the ability to work effectively in a fast-paced, team-oriented environment.
The ideal candidate will demonstrate cultural humility and the ability to provide trauma-informed, patient-centered care to individuals from varied backgrounds and age groups.
Comfort working with common outpatient diagnostic presentations—such as mood disorders, anxiety disorders, trauma-related conditions, and co-occurring substance use—is important.
