EnableU is a Sydney-based provider delivering person-centred care across aged care, disability and community health. As a Support at Home provider, we help older Australians live independently, safely and well in their own homes, on their terms.

We're growing our aged care team and looking for a compassionate, experienced Case Manager/Care Partner who speaks Macedonian to support our older clients across Sydney.

The Opportunity

This is a client-facing role where you'll lead care coordination under the Support at Home program, manage individual support plans and make a real difference to the people you support. You'll work in a close-knit, flat-structure team where you're trusted to work autonomously while ensuring clients receive the right services at the right time.

The ability to speak Macedonian is a genuine requirement of this role. Many of our clients and their families are Macedonian speakers, and being able to communicate with them in their own language is central to delivering safe, respectful care.

Key Responsibilities

- Build strong, respectful relationships with clients, families, support workers and external providers

- Conduct assessments and develop, implement and review care plans

- Coordinate tailored services that reflect each client's goals, health conditions and cultural needs

- Make decisions that prioritise client safety, wellbeing and quality of life

- Lead and supervise your team of Support Workers to ensure service quality and continuity

- Monitor and manage client budgets to ensure full and appropriate use of Support at Home funding

- Maintain compliance with the Aged Care Act,



the strengthened Aged Care Quality Standards, funding guidelines and internal policies

- Document accurate, timely progress notes on client interactions, changes in condition, incidents and follow-up actions

- Review notes from support workers, suppliers and allied health professionals to keep care coordinated

- Manage intake, admissions and meet and greets for new clients

About You

- Able to communicate confidently in Macedonian (essential), with strong English

- Proven experience in aged care, home care, community services or a health setting

- Sound assessment, care planning and case management skills

- Confidence to work independently and manage competing priorities

- Excellent communication, relationship-building and leadership skills

- Working knowledge of My Aged Care, Support at Home and the Aged Care Quality Standards

- Current National Police Check (or willingness to obtain)

- Driver's licence, own vehicle and willingness to travel across Sydney

- A clinical background in nursing or allied health is a bonus, not a requirement
