Job responsibilities:
Perform reassessments and update the plan of care based on patient response.Work 40 hours per week with 8-hour shifts.Communicate functional changes and barriers to the care team to support care planning.Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate.
